Click on the button then from the new browser window click the 2nd icon on the upper right-hand corner to download.

IMPORTANT INFORMATION!

Registration Form uses PDF form technology.  You must have a PDF Reader installed in order to fill and email installed to return the form.  If you do not already have such a reader, it is available on the internet: Adobe Reader

For the form to work properly (including auto email return), you must first save it onto your computer, and reopen it with Adobe Reader DC to add information.  Then resave and submit form once completed.
